El Sol Medical Focus Day Program

El Sol Medical Focus Day program strives to maintain an individual's optimal health, improve body strength, and encourage self-empowerment. The program provides supports for individuals with restricted health care plans or chronic medical health service needs. Individuals participating in the program have a diagnosed medical and/or physical condition that requires a greater intensity of nursing care and medical intervention. Individuals in this program may have multiple physical disabilities and nursing care needs. Those served in this program require assistance in daily living and ongoing management of chronic and multiple healthcare conditions. Individuals served may require assistance in all areas of personal care and may require the use of adaptive equipment and/or staff support/assistance for transfer and mobility needs. Through providing personalized supports we aim to maximize an individual's participation in community integrated activities that include work, volunteerism, and socialization.

The program may provide services in either our site based facility or an individual’s home. If an individual elects to receive our services within their home, our instructors will work to foster better health and socialization habits in order to allow participation in a community based program. Instructors provide daily health monitoring, medication management, testing, and physician prescribed treatments. Through body awareness and movement instruction we will assist individuals in improving their range of motion, muscle coordination, and circulation.

The program will provide individuals with an opportunity to showcase their talents and express themselves through their personal art projects. Individuals will participate in community integration through volunteerism and job readiness training. El Sol's support team will work towards strengthening an individual's ability to successfully participate in a variety of work and social experiences.
